Scheduling an Appointment

Impact Psychiatry PC offers ONLY Virtual visits to Arizona Residents. Services are rendered to Child, Adolescent, Adult and Older Adults ages 8– 70 years old.

To establish care with us, call the office or click on schedule appointment to complete a short intake form. Upload your insurance card if applicable. Upon receiving your request, and approved as a patient, you will receive an email with a link to access the patient portal provided by our HIPPA secured electronic health system to self- schedule your appointment, (or call the office), review all intake forms, sign and submit. No download is required. Patient portal for established patient can also be accessed on our website. We have streamlined our secured intake process to make it easy for clients to access forms and join Zoom Health virtual visits via any computer, IPADs, Tablets or phone.

We do not offer walk-in appointments. Visits are by scheduled appointments only. If having an emergency, call 911 or go to the nearest Hospital or urgent care for evaluation.

Insurance and Fees

We accept the following insurance:

• Cigna
• United Healthcare/Optum
• Aetna
• BCBS

All other services rendered will be self-pay.

For cash pay clients: We accept all major Credit Cards or Zelle payments to: info@impactpsychiatry.com. Please call for sliding scale for self-pay clients.

Out-of-network patients can be seen as self-pay but we do not provide superbills to cash pay patients.
