Scheduling an Appointment

Impact Psychiatry PC offers Telemedicine visits to New York Residents only and by appointment. No walk in appointments. To establish care with us, click on “Schedule an Appointment”. Upon receiving this request, you will receive access to the “Patient Portal” provided by our electronic health system. A secured patient portal link will be sent to your email same day to create your “patient portal account” or Click Here to create portal and sign all required patient consent forms prior to your scheduled appointment. You are required to upload your New York State Driver License or New York State ID and Insurance card (if insured). Signed patient consents and all required forms must be received in order to establish care with us. If having an emergency, call 911 or go to the nearest Hospital for care.

Payment at Time of Service Policy

  • We require that clients make full payment at the time of services. You will be provided with information about your insurance benefits for services requested prior to scheduling, as well as information about private pay rates.
  • We require that a credit card be on file.

Cancellation and No Show Policy

  • We require clients to provide one full (24hrs) business day notice of the cancellation of their appointment. If your appointment is on a Monday, it must be cancelled by the Friday.
  • Clients that are 15 minutes or more late for a 30 minute appointment or 20 minutes or more late for a 60 minute appointment will be considered a “no show” and billed for the service as providers do not have adequate time for treatment.
  • If indications of child abuse, elder abuse, or planned bodily harm to oneself or to others come to light in the course of treatment, the providers are legally bound to report these to the appropriate authorities. Clients are required to sign a statement of understanding of their rights to privacy, which will be provided via Patient Portal.
  • Client's card on file will be charged $100 for missed appointments or late cancellations – this may exceed the insurance company rate. Please note, insurance companies will not pay for missed sessions; payment for these sessions are the responsibility of the patient and must be taken care of before a client can be rescheduled.

Right to Terminate

Impact Psychiatry PC reserves the right to terminate the provider-patient relationship under the following circumstances:

  • When services may no longer be beneficial to the client
  • When another professional might be of better service
  • When payment is not received
  • When a client misses two consecutive appointments without 24 business hours notice or frequently misses appointments
  • When a client does not cooperate with the agreed upon treatment
  • If a client or family member/significant other are hostile or aggressive, or cause any disruption in our workspace

Confidentiality

  • Impact Psychiatry PC takes safeguarding the client’s information extremely seriously. Impact Psychiatry PC is bound by law to protect client confidentiality. Any disclosure of your treatment to others will require your explicit written consent. However, Impact Psychiatry PC providers are mandated reporters.
  • If indications of child abuse, elder abuse, or planned bodily harm to oneself or to others come to light in the course of treatment, the providers are legally bound to report these to the appropriate authorities. Clients are required to sign a statement of understanding of their rights to privacy, which will be provided via Patient Portal.

Controlled Substance Policy and Ryan Haight ACT

Due to the Public Health Emergency caused by COVID-19 pandemic, The Ryan Haight ACT was amended to allow prescription of controlled substance via Telemedicine. This is one of the exceptions of the Federal Ryan Haight Act’s requirement to conduct an in-person exam before prescribing controlled substance via Telemedicine. As an established client of Impact Psychiatry PC, You will be required to have an in-office physical visit once per year if the Ryan Haight Act goes back into effect for continued prescription of controlled substance.

Benzodiazepine Policy

Scheduling an Appointment

Benzodiazepines (medications such as Ativan, Klonopin) when prescribed, it will be done monthly. No 90 days of any controlled substance will be prescribed. In addition, benzodiazepines carry the risk of cognitive decline over long term use, risks of tolerance, dependence and abuse. Benzodiazepine are potentially lethal in overdoses, particularly when mixed with alcohol and/or opioid pain medications. Client must also agree to routine drug test  and as needed. If positive for THC or other illegal drugs, client will be weaned off of Benzodiazepine. Lost or stolen prescription will not be refilled till the next due 30 days period. Using more than prescribed will result in termination of patient-Provider relationship.

Stimulant Policy

Stimulant medications (Such as Adderall, Ritalin) are a first line treatment for the symptoms of ADHD, however, they carry the risk of tolerance, dependence and abuse. For this reason, their use is tightly regulated. when prescribed, it will be done monthly. No 90 days of any controlled substance will be prescribed. Client must also agree to routine drug test and as needed.  If positive for THC or other illegal drugs, Stimulant will be discontinued. Lost or stolen prescription will not be refilled till the next due 30 days period. Using more than prescribed will result in termination of patient-Provider relationship. Clients are required to obtain Blood Pressure monitor and check Blood pressure readings day of scheduled appointment.

Refill Policy

The routine practice of Impact Psychiatry PC is to write a prescription to cover your medication needs until your next appointment. If you keep scheduled appointments or reschedule promptly, there should be no need for additional refills. If an exception occurs, please call the office to request a refill at least 5 working days before you will run out of medication. Refills will be considered on a case by case basis. Please note, we do not process refill requests after hours, on weekends or on holidays. Dose changes will not be made outside of scheduled appointments. Please allow 48-72hrs to process medication prior authorizations. 90 day medication supply will not be filled on initial visit.

Once you become an established patient and required by your insurance company, Impact Psychiatry PC will fill a 90 day supply for non-controlled prescriptions but clients must still keep to their scheduled appointments. If you cancel or re-schedule your appointment because you have a 90 days prescription, subsequent requests will be denied.

Please OPT OUT of automatic refill requests if offered this service by your pharmacy. We will not respond to pharmacy- initiated refill requests due to the significant administrative burden this entails.

Please remember that it is your responsibility to schedule a follow-up appointment before you run out of medication and within the return time frame determined in your treatment plan. Controlled substances cannot be refilled by phone and will only be written during office visits if appropriate. There will be no early refills for controlled substances regardless for the reason for the request.

Limitations on Requests for Forensic and Court Support Services

  • Disability: We do not perform disability evaluations. Clients can request a copy of their medical records to be sent to their disability Attorneys if they choose to. FMLA will only be granted for a short period of time not to exceed 12 weeks while client is actively receiving psychotherapy and medication adjustment at the clinic.
  • Emotional Support Animals: We do not offer assessment or report writing for primary purpose of gaining an emotional support animal letter. We provide advocacy and documentation to established clients who have been working with us for at least three months and who have attended all scheduled appointments.
  • Parenting evaluations: We do not perform parenting evaluations and we will not appear in court to testify on your behalf. If we receive a subpoena to appear in court on your behalf, our services will be billed at our hourly rate of $1000 per hour.
  • Paperwork: We bill an hourly rate of $75/hour for all documentation services provided to our clients outside of treatment sessions.

Insurance and Fees

We are currently in the process of credentialing with Empire Blue Cross Blue Shield and Magellan. As such we are not accepting insurance at this time. All services rendered will be self-pay. We will update our insurance information once credentialed.

  • Initial Psychiatric Evaluation: $180
  • Follow-up Med Check: $85
  • Psychotherapy 30mins: $105
  • FMLA Paperwork: $100

Out-of-network patients can be seen as self-pay but we do not provide superbills to cash pay patients.

Impact Psychiatry PC requires your credit or debit card to be kept on file as a convenient method of payment and for the portion of services that your insurance doesn’t cover, but for which you are liable. Without this authorization, we will not be able to provide services for you.

Your credit card information is kept confidential and secure, and payments to your card are processed immediately for self-pay patients and for patients with insurance, only co-payments will be charged at time of service. After the claim has been filed and processed by your insurer, and the insurance portion of the claim has been paid and posted to the account, your card will be charged for any outstanding balance not paid by your insurance if applicable. Payment will be expected within 30 days to avoid further collection costs.
